1. Complete the form using the information below:

   Particulars: Your name (up to 12 letters)
   Code: Project number (if giving to a specific Project)
   Reference: Project name (first 12 letters)

2. Tell us your name and address for an annual receipt. To enable us to link your payment(s) with you, please also include
   - for a Direct Credit - the transaction date and amount
   - for an AP (Automatic Payment) - the start date, frequency and amount

2. Please include the following information:
   - The project number and name, or other purpose of the donation
   - Your name and address so we can send an annual receipt.

3. Post to the address above.
