To use the AP paper form, obtain a copy from your bank and then complete both steps under Electronic Banking.

Complete the form using the information below:

      4. Particulars: Your name (up to 12 letters)
      5. Code: Project number (if giving to a specific Project)
      6. Reference: Project name (first 12 letters)

Tell us your name and address for an annual receipt. To enable us to link your payment(s) with you, please also include:

1. 1. 1. for a Direct Credit - the transaction date and amount
      2. for an AP (Automatic Payment) - the start date, frequency and amount

Cheque / Money Order - (Please do not send cash through the post.)

2. Please include the following information:
   • The project number and name, or other purpose of the donation
   • Your name and address so we can send an annual receipt.
